PAES Counseling and Pre-Vocational Services Rotation

A. Learning Goals / Competencies

  1. Conduct initial intake assessments:
    A.
    Collect client history including history of presenting issues, past mental health and substance abuse histories, employment history and review of barriers to current employment, assessment of psycho-social context ;
    B.
    Perform Risk Assessment
    C.
    Formulate a Provisional Diagnosis
    D.
    Complete all required documentation.
    E.
    Schedule client for follow-up program orientation with Intake Coordinator.
  2. As indicated, carry a caseload of one longer term therapy client with the goal of addressing the client’s barriers to employment and increasing access to resources for self-sufficiency.
  3. Conduct or co-facilitate a therapy group (either structured harm-reduction based substance abuse group, or process oriented group using creative expression to address mental health issues).
  4. Identify and understand the psychological needs of individuals who suffer from a combination of severe mental illness, substance abuse issues, unemployment, homelessness and poverty.
  5. Gain knowledge of resources available for discharge planning and follow-up care.
  6. Learn the structure and feature of a complex welfare to work organizational context and its place in the overall adult system of care.

B. Training

  1. Participation in PCS Clinical Meeting one and a half hours per week. Content involves: Case conferences, clinical updates, psychiatric consultation, and trainings on various clinical topics.

  2. Weekly clinical supervision with Dr. Hillary Weiss, Psy.D.

  3. Ongoing on-site consultations with Behavioral Health Counselors, Vocational Rehabilitation Counselors, Clinical Supervisors, Case Manager, and Psychiatrist on assigned cases and general clinical issues.

  4. Administrative supervision and oversight provided by Debra Salan, MSW, Ph.D., Program Director.

C. Schedule

Two rotation placements are available; one September through February and the other during March through August. Rotation day is Thursday; hours are 8:30-5:00.

Abraham Lincoln High School

Rotation

A. Responsibilities and Learning Goals
  1. Carry a caseload of three adolescent clients maintaining on-going therapeutic contact within the school setting, including collateral parent sessions and school personnel consultation sessions as needed.
  2. Conduct or co-facilitate a time-limited therapy group (topic to be determined between intern and on-site staff).
  3. Complete intake assessments with students referred to Wellness Center (at least one per week).
  4. Intervene clinically with at-risk students in the context of the school milieu.
  5. Become familiar with the policies and procedures of Early Prevention, Screening, Diagnosis, and Treatment (EPSDT) program, the largest federally funded program for high risk youth covered by MediCal, with the possibility of taking on such cases. 
  6. In collaboration with other school stuff (teacher, academic counselors, staff of the multidisciplinary Wellness Team and school-based clinicians.  
  7. Become skilled at applying clinical acumen within the educational model view of adolescents and the mental health perspective. Learn to use this knowledge in developing clinically appropriate interventions within the school milieu. 
  8. Learn to identify and understand normal developmental conflicts of adolescence and to differentiate them from more severe problems that require clinical interventions.
B. Training
  1. On-site weekly clinical consultation and administrative supervision with Sheening Lin, M.S., Coordinator of Abraham Lincoln High School Clinic.
  2. Weekly clinical consultation meetings for the clinical staff working at the school. 
  3. Option to attend clinical group supervision with Kristin Chun, LMFT on drop-in basis (Wednesday afternoons). 
C. Schedule

One rotation placement is available: September through the end of May.  Work hours are Thursdays, eight hours per week.

San Francisco General Hospital

Psychiatric Emergency Services

Rotation

A. Learning Goals/Competencies
  1. Conduct clinical work as a member of Psychiatric Emergency Services treatment team including participation in staff meetings.
  2. Conduct diagnostic crisis interview:
        A. Collect detailed history including history of present illness & past histories; review of
             old charts; collateral information from family, outpatient providers, etc.
        B. Perform detailed Mental Status Exam
        C. Formulate a Diagnosis
        D. Formulate a Plan of Care
  3. Present case to Attending Psychiatrist.
  4. Following interview of patient by Attending Psychiatrist implement plan of care (except medical and nursing components - physical exam, lab interpretations, Med Rx; med administration, vital signs, management of seclusion and restraint)
  5. Become skilled at crisis assessment and intervention within the emergency services environment.
  6. Develop close familiarity with the laws, policies and procedures involved in involuntary hospitalization.
  7. Gain knowledge of resources available for discharge planning and follow-up care.
  8. Learn the structure and features of psychiatric emergency services programs and their role in the overall system of care for the mentally ill.

B. Training

  1. Participation in a PES Staff Meeting one hour per week. Content involves: Case Conferences, Clinical Updates, Pharmacology Updates, and Administrative Matters.
  2. Bob Buckley, MD, Attending Psychiatris, provides ongoing on-site clinical supervision.
  3. Ongoing on-site consultations with other Attending Psychiatrists and clinical staff on assigned cases. 
C. Schedule

Two rotation placements are available.  September through February and March through August. Rotation day is Thursday; hours can be arranged to match either the morning or the afternoon shift (both shifts are 8 hours).

Broderick Street Adult Residential Facility

Rotation

A. Learning Goals
  1. Conduct clinical work and staff consultation as a member of a multidisciplinary residential treatment team, including participation in staff meetings, treatment planning, and clinical case conferences.
  2. Interview patients and team members and provide psychosocial assessments in a residential, inpatient-like environment.
  3. Learn differential diagnosis in complex cases; recognize symptoms and functional impairments related to mental, physical, and substance abuse problems.
  4. Identify and understand the psychological needs of individuals who suffer from a combination of severe mental illnesses, disabling medical conditions and substance abuse issues.
  5. Learn to develop assessment-base behavioral modification plans suitable for residential treatment. 
  6. Become skilled at applying clinical understanding within the residential care milieu and providing practical treatment consultations to multidisciplinary treatment staff.
  7. Conduct or co-facilitate a time-limited therapy group (topic to be determined between intern and on-site staff).
  8. Understand the benefits and features of residential care programs and their role in the overall system of care for clients with sever and persistent mental illnesses.

B. Training

  1. Broderick Street Adult Residential Facility Team Meetings, one hour per week. Content involves: Case Conferences, Didactic Presentations, Clinical Updates, and Administrative Issues.
  2. Off-site consultation on assessment issues with Anastasia Glezerman, Ph.D., one hour biweekly.

  3. Ongoing weekly clinical and administrative supervision at the rotation site provided by Michael Badolato, LMFT, Program Director

C. Schedule

Rotation placement is June through August, with an option of carrying selected rotation responsibilities (i.e. group facilitation) throughout the training year.  Hours are flexible - depend on duties, eight hours per week on average.
